Wi-Fi Camera Installation Disclosure & Service Agreement
This Wi-Fi Camera Installation Disclosure & Service Agreement (“Agreement”) is entered into between Mid Century Fiber (“Provider”) and the undersigned customer (“Customer”). This Agreement applies to the Customer identified on the service order, including customer name, service address, contact information, and installation date associated with the Wi-Fi camera installation.
Terms & Conditions
- Scope of Service. Mid Century Fiber will install Wi-Fi camera equipment at the Customer’s residence at the Customer’s request. Installation services may include hardware installation, basic configuration, and connection to the Customer’s internet service. This Agreement does not include professional monitoring, emergency response services, alarm verification, or notification of law enforcement or other emergency responders.
- Term of Agreement. This Agreement has a minimum term of twelve (12) months beginning on the installation date. Upon completion of the initial twelve-month term, the Agreement will automatically convert to a month-to-month agreement unless canceled by the Customer in accordance with Section 9 of this Agreement.
- Early Termination Fee (ETF). If the Customer cancels service prior to completing the initial twelve-month term, an Early Termination Fee (ETF) will The ETF shall be equal to either a specified flat fee or the remaining monthly charges due under the initial term, whichever is less. The applicable ETF will be billed on the Customer’s final invoice and is due immediately upon issuance.
- Customer Premise Equipment (CPE) All Wi-Fi cameras, related hardware, and associated equipment installed as part of this service (“Customer Premise Equipment” or “CPE”) are and shall remain the sole property of Mid Century Fiber at all times. Ownership of the CPE does not transfer to the Customer under any circumstances.
- Return of Equipment Upon Cancellation. Upon cancellation of service for any reason, the Customer agrees to return all CPE to Mid Century Fiber in good working condition, normal wear and tear. All equipment must be returned within seven (7) business days of service cancellation. Failure to return equipment within this timeframe may result in unreturned equipment charges equal to the full replacement cost of the CPE.
- Customer Responsibilities & Legal Compliance. The Customer acknowledges and agrees that they own and control all camera accounts, settings, recordings, and access permissions associated with the Wi-Fi camera system. The Customer is solely responsible for complying with all applicable federal, state, and local laws governing video and audio recording.
The Customer further acknowledges that the Wi-Fi camera system utilizes third-party hardware, software, mobile applications, and cloud-based services provided by Eufy. Use of these services is subject to Eufy’s Terms of Service and related policies. By activating or using the Wi-Fi camera system, the Customer agrees to be bound by Eufy’s Terms of Service, which are available at: https://www.eufy.com/policies/terms-of-service
Illinois-Specific Consent Notice: The Customer acknowledges that Illinois is a two-party consent state under the Illinois Eavesdropping Act (720 ILCS 5/14-1 et seq.). Audio recording of conversations is prohibited unless all parties to the communication have provided consent, except where otherwise permitted by law. The Customer agrees not to enable or use any audio recording features unless legally permitted and all required consent has been obtained. The Customer further agrees to provide any legally required notice or signage related to video recording.
The Customer confirms that all camera placement locations have been reviewed and approved by them and understands that cameras should not be installed in areas where individuals have a reasonable expectation of privacy, including but not limited to bathrooms or private living spaces, unless explicitly permitted by law.
- Internet, Power & Performance Disclaimer. Wi-Fi camera functionality is dependent upon reliable internet service, adequate Wi-Fi coverage, continuous electrical power, and the availability of third-party applications or cloud-based services. Mid Century Fiber is not responsible for service interruptions, recording failures, data loss, or degraded performance caused by internet outages, Wi-Fi limitations, power failures, or issues related to third-party platforms or applications.
- No Monitoring & Limitation of Liability. Mid Century Fiber does not monitor live camera feeds, recorded footage, or system alerts. Mid Century Fiber is not responsible for missed events, unauthorized access, hacking incidents, or device failures occurring after The Customer assumes all risk related to the use and operation of the Wi-Fi camera system.
- Cancellation After Initial Term. After completion of the initial twelve-month term, the Customer may cancel service with proper notice in accordance with Mid Century Fiber’s standard cancellation policy. No Early Termination Fee will apply after the initial term has been fulfilled, provided all equipment is returned as required.
- Customer Acknowledgement & Consent. The Customer acknowledges that Mid Century Fiber technicians do not determine camera placement, recording settings, audio enablement, or legal compliance and act solely at the direction of the Customer. The Customer further acknowledges that use of the Wi-Fi camera system requires acceptance of third-party terms and conditions, including Eufy’s Terms of Service, and confirms that they have been provided the opportunity to review such terms prior to activation.
REOLINK IP CAMERAS RESELLER’S TERMS AND CONDITIONS
These Terms and Conditions (“Agreement”) govern the purchase and use of Reolink IP Camera systems (“Equipment”) sold to residential customers by Mid Century (“Company,” “we,” “us,” or “our”). By purchasing and using the camera(s), you (“Customer”) agree to the following terms:
1. SERVICE DETAILS
1.1 Product and Ownership
- Ownership turns over to customer after 12 month commitment period.
1.2 Support & Maintenance
- Initial setup assistance is provided by Company governed by a separate installation agreement.
- Customer is responsible for routine maintenance.
1.4 Compliance with Reolink Terms
- Customer agrees to comply with all applicable Reolink terms and conditions regarding the use of the Equipment, software, and services. Reolink’s terms and conditions can be found at https://reolink.com/terms-conditions/.
- Any violation of Reolink’s terms resulting in service disruption or warranty issues is the sole responsibility of the Customer.
2. USAGE POLICIES
2.1 Proper Use
- Customer must use the Equipment in compliance with all applicable laws and regulations.
- The Equipment must not be used for unlawful surveillance, invasion of privacy, or any activity violating third-party rights.
2.2 Network & Security Responsibility
- Customer is responsible for securing their network and implementing adequate security measures to prevent unauthorized access.
- Company is not liable for security breaches due to inadequate Customer security measures.
2.3 Restrictions
- Customer may not modify, tamper with, or reverse-engineer the Equipment.
- Unauthorized resale or redistribution of the Equipment is prohibited.
3. FEES AND PAYMENT TERMS
3.1 Equipment Purchase
- The Camera Package is billed at $40 per month for 12 months. Equipment is owned by the customer once the 12-month commitment has been fulfilled.
3.2 Installation Fees
- Any installation fees shall be governed by a separate installation agreement.
4. WARRANTY AND LIABILITY
4.1 Manufacturer Warranty
- The Equipment is covered under the Reolink manufacturer’s warranty.
- Warranty claims must be directed to Reolink per their policies.
- Customer agrees Company has made no warranties regarding the Equipment.
4.2 Limitation of Liability
- Company is not liable for indirect, incidental, or consequential damages arising from the use or inability to use the Equipment.
- Company is not responsible for data loss, security breaches, or unauthorized access to recorded footage.
- If it is determined Company is liable, Customer agrees the amount of damages shall not exceed the monies Company received as a reseller of the Equipment
5. TERMINATION AND MODIFICATIONS
5.1 Service Termination
- Customer may discontinue Equipment usage at their discretion. Additionally, services provided by Company shall be governed by an installation service agreement.
5.2 Modifications to Terms
- Company may update these terms and conditions periodically.
6. GOVERNING LAW
- This Agreement is governed by the laws of the State of Illinois. Customer agrees any claims asserted against Company shall be brought in Fulton County, Illinois Circuit Court.
7. INTELLECTUAL PROPERTY
- All Reolink trademarks, logos, brand names, software, and copyrighted materials associated with the Equipment are the exclusive property of Reolink and/or its licensors.
- Customer is granted a limited, non-exclusive, non-transferable license to use the software and firmware embedded in the Equipment solely for its intended purpose.
- Unauthorized reproduction, modification, distribution, or commercial use of Reolink’s intellectual property is strictly prohibited.

By purchasing and using the Equipment and services, Customer acknowledges that they have read, understood, and agreed to these Terms and Conditions, including Reolink’s Terms and Conditions.
